Is study retrospective or prospective / combined because in six month duration age 0-24 months children will not covered prospectively? ==== its primarily retrospective, based on recall history. you will preferably involve children of age 15-18 months who will come for their MMR or booster 1 vaccine. Also you will follow the course of the present vaccination, thus making it a combined study.
2. Is there two objective of study first to assess the compliance? to assess the adverse event? ==== yes. both based on history associated with each immunisation, the dates and the adverse events with current immunisation.
3. Should questionnaires of each vaccine separate? it should be under separate headings.
4. Are Questionnaires for adverse event separately prepare? same questionnaire but with a separate heading.
5.Is that possible to prepare questions according what time vaccine has given like birth , 6, 10, 14 week 9months, 16-24months and assess their compliance separately?= == time / age of vaccination is likely to be available on records. try to recall dates, ask for immunisation card, if not available, amke a note of it. check the delay in number of days associated with each vaccination and evaluate the possible reasons for it..

statistical analysis, will primarily be descriptive, to analyse the compliant and noncompliant data by socioeconomy, education and residence of parents close to a facility for immunisation. It will also, derive the incidence and prevalence of adverse events. various factors amongst compliant and noncompliant group will be analysed by modified T test.
also write in questionnare:
adverse events: to which vaccine? 1given at birth BCG 2.given at 6,4,10 weeks or DPT,HBV 3.given at 10 months ?measles.. etc
so prepare questions related to individual vaccine side effects also.
reasons for no vaccinating as per schedule:
1. medical/fever 2. far place 3. natural.. rains etc 5. not so serious 6. told by a doctor 7. other
